立法會口頭質詢:《香港與泰國的跨境二維碼支付互聯》(2024.04.10)

陳健波議員:多謝主席。主體答覆指出,香港於今年1月已躋身泰國跨境二維碼支付交易量的第三位,證明這種支付方式深受港人歡迎。局長剛才表示,金管局會繼續研究將這種支付方式推廣至其他地區,而我相信,這麼好的支付工具應該加以推廣。我的補充質詢 是,下一個目標是哪個地區呢?時間表為何呢? 財經事務及庫務局局長:多謝主席,亦多謝議員的補充質詢。正如 我剛才提及,我們會以現行經驗作為參考,而事實上,我們亦有與不同地區討論交流,研究將有關支付方式推廣至當地的可行性。至於時間表為何,以及哪些地區優先,由於牽涉其他地區或央行與我們的討論,恕我現階段無法答覆。不過,當時機合適,可以透露時,我們會盡快向立法會交代。多謝主席。

立法會書面質詢:《減少浪費藥物》(2024.04.10)

陳健波議員:據報,醫院管理局(“醫管局”)早前表示,該局每年用於藥物的開支逾96億元,佔其總開支約一成,而該局將進一步 檢討藥物配發及收費制度,以減少不必要的浪費。就此,政府可否告知本會: (一) 會否檢討公營醫療機構配發藥物的情況,例如研究向接受專科門診服務的病人配發每種處方藥物16周劑量的上限是否需要改變; (二) 鑒於據報,部分病人(特別是老年病人)的家中囤積大量藥物,政府有否檢視,是否存在公營醫療機構過度配發藥物,或有關病人未有按時服藥的問題;如有檢視而結果為是,有何解決方法; (三) 鑒於據報,因應2019冠狀病毒病第五波疫情爆發時導致藥物供應緊張,醫管局把配發每種處方藥物的劑量減至4周,政府有否檢討有關安排的成效,以及會否建議醫管 局把該安排恆常化;及 (四) 鑒於據悉,坊間曾有機構推出回收藥物計劃,收集病人剩餘或過期藥物,當局會否考慮參考有關經驗,向病人收集 有關藥物;如會,詳情為何;如否,原因為何? 醫務衞生局局長 主席:  香港公營醫療服務獲政府大幅資助(佔成本 97%以上),以 確保市民不會因經濟原因而無法獲得適當的醫療服務。本港人口持續老化、慢性疾病日益普遍,導致醫療需求日益增加。為配合公營醫療服務需求增長,醫院管理局(醫管局)需要每年投放足夠資源予高額資助的醫療服務及藥物供病人使用。因為藥物成本上升、以臨床實證為本為病人引進所需的新藥物,以及擴闊藥物名冊等措施,醫管局設有既定機制,監察藥物的使用及開支情況,務求謹慎使用有限的公共醫療資源。 就陳健波議員的提問,經諮詢醫管局和環境保護署(環保署) 後,現綜合回覆如下: (一)至(三) 病人求診時,醫生會根據專業診斷及病人的臨床情況處方合適和適量的藥物。醫院藥劑部職員亦會在配發藥物給病人時,提醒病人應按照標籤上的指示服藥,避免自行加減服藥的次數或藥物的份量。醫管局一直致力提供藥物輔導和藥物評估及整合服務,以及不斷優化現行公立醫院配發藥物的機制,確保病人安全用藥及加強病人用藥的依循程度,同時避免病人在家中儲存大量藥物,從而減低誤用風險,以及減少藥物過剩形成浪費。 為提高醫囑服藥成效及減少不必要的藥物浪費,醫管局於二 〇〇三年實施專科門診藥物收費,當時以每種處方藥物最多16星期為一徵收單位,每單位收費為港幣10元。於二〇一七 年收費檢討後,每單位收費增加至15元。普通科門診的診金亦於同年增至50元,費用已包括不限種類數目的處方藥物在內。政府及醫管局會按既有機制,定期檢討包括藥物收費在 內的公營醫療服務收費和安排。在現正進行新一輪的服務收費檢討工作中,醫管局會循鼓勵病人恰當使用藥物方向考慮,檢視藥物費用按量按期收費模式,並向政府提交方案。 政府考慮檢討建議時,會根據適用的檢討原則(包括考慮成 本、市民負擔能力、服務是否恰當使用、資源運用的優次、扶助弱勢社羣以及公眾的認受等因素),顧及建議對不同負擔能力市民帶來的影響,亦會關注現時資源分配失衡情況比較明顯的服務,以及有出現資源浪費的情況,需否透過調節收費安排,以誘導市民大眾善用公營醫療服務,並同時進一 步支援有確切需要人士包括弱勢社羣和嚴重病者,以確保有限的資源用得其所。政府及醫管局會適時公布檢討結果。 此外,為了提升用藥安全,醫管局自二〇一七年十二月開始, 逐步在公立醫院推行「覆配易」覆配藥物服務。醫院藥劑部會將參與「覆配易」服務的病人的藥物處方分成較少的數量 和容易處理的包裝,分期配發藥物,並安排藥劑師在覆配藥 物前覆核藥單和確定病人的最新情況。服務目前於15 間公立醫院推行,涵蓋大約十萬名病人。醫管局近年亦投放額外資源設立藥劑師診所,主動為有需要的專科病人提供臨床藥劑服務,例如為病人提供全面的藥物輔導及評估,協助病人或其照顧者選擇和正確服用藥物,以提升病人服藥依從性及減低藥物過剩的風險。 在 2019 冠狀病毒病疫情期間,部分藥物供應出現不同程度的中斷和不確定性,其中,市民對藥物「撲熱息痛」的需求 一度急增。為免市民不必要地儲存過量藥物,醫管局就藥物「撲熱息痛」作分階段配發安排,以每次最多四星期份量為限,病人如有臨床需要,可覆配有關藥物。上述安排實施後, 二〇二三年第一至第三季平均約六成半病人未再返回醫院覆配藥物「撲熱息痛」。上述經驗顯示恰當的藥物按量按期配發安排,對推動市民妥善使用藥物(特別是需要時服用的藥物)有正面作用。政府考慮醫管局的檢討建議時,會參考過往 的相關經驗。 (四) 從環境角度,由於家居棄用的藥物一般數量不多,現時是與一般家居固體廢物混合處理。環保署對於香港堆填區滲濾污水的處理及排放程序有嚴格的標準,根據一項針對環境水體的科學研究結果,殘餘藥物所引致的環境影響極為輕微。環保署現時未有計劃設立藥物回收安排。 醫管局會繼續與相關部門、病人組織、藥劑專業團體及不同 持份者溝通,透過教育、宣傳及行政措施,持續優化配發藥物機制,以及減少病人剩餘藥物的情況。醫管局會密切監察藥物使用情況,確保病人得到適切治療的同時,資源運用高效且具成本效益。
