《全面推展醫療融資改革,善用公私營醫療資源》議案發言(2025.06.12)

多謝主席。多謝林哲玄議員提出今日的議案以及其他議員的修正案。香港醫療融資體系改革這一議題近年討論不絕,2019年政府推出自願醫保計劃,可謂醫療改革進程的關鍵突破,截至去年3月,保單數目已超過134萬張。 本港醫療融資結構主要由3部分組成。根據2022-2023年度的數據,公營醫療系統佔整體醫療開支的58%,“用者自付”部分約佔 27%,商業醫療保險佔15%。政府在3月提出公營醫療收費改革,其後在5月亦建議就私營醫療收費透明度立法,加上檢討自願醫保保障範圍等,3項措施都標誌着醫療融資改革的重要里程碑。 政府宣布啟動公營醫療收費架構檢討,透過改革資助架構,引導善用資源,提升服務效率。收費改革後的資助率預料由大約97%下降至5年後的90%,有助提高公營醫療可持續性。另外,公營醫院增加收費的同時,亦優化費用減免機制,放寬收入和資產限額,令 部分以往不獲減免的市民,在收費改革後獲得全額減免,加上增設全年收費上限1萬元,加強對危重病人的藥械保障,預計合資格受 惠人數會由現時的30萬人大幅增至140萬人,惠及更多市民,令安 全網更加穩固。 另一方面,我多次強調現時私營醫療收費增幅驚人,醫療通脹增幅遠超一般通脹。高昂的醫療收費令保險公司面臨的賠付壓力大 增,最終直接推高保費,令部分中產家庭難以負擔,加上私營醫療收費透明度不足,同一手術由不同醫生或私家醫院處理,其差價可 以很大,令市民即使有醫保也擔心賠款不足,自己要承擔巨額醫療 費用。如果情況不改變,中產在面對保費飆升的情況下,都只能被迫放棄醫療保險,重返公營體系,加劇醫療負擔,恐怕令輪候時間進一步延長。 針對私營醫療市場價格欠缺透明度的現象,政府成立有全面醫 療專業代表性的“醫卓所”,制訂公私營通用的臨床指引,減少不必要的檢查程序及手術,因為不當醫療手法不但昂貴,相關風險亦可能危害病人。未來要規定,私營醫療機構提供服務費用預算時,如 果最終收費超出預算20%或以上,需要向病人提供書面解釋,以保障各方權益。未來也會有收費比較平台,方便市民比較不同私家醫 院之間的收費。 值得注意的是,香港的醫療業界真的要明白醫療改革勢在必行。 近年內地醫療體系正急速改變,深圳三甲醫院牙科價格大概只是香港私營診所的三分之一,而且質素不差,吸引大批港人北上看牙醫。 有內地牙科機構指出,每年接待超過10萬名香港病人,佔所有病人 的一半。如果本港醫療界仍然維持現有模式,收費高昂而且透明度低,現時大灣區來往如此方便,恐怕大量市民將轉往內地求醫。香港醫療業要醒覺,必須主動公開價格,維持合理利潤,繼續保持專業嚴謹的態度,令用家覺得物有所值,方能保住香港醫療的競爭力。 多謝主席。
